Giuseppe Remuzzi, director of the Mario Negri Institute in Italy, after Donald Trump's warning that the use of paracetamol during pregnancy risks causing autism, said that there is no increased risk for this thesis.

According to him, paracetamol remains the safest medicine.

"Talking about an increase in the number of cases is absolutely incorrect. However, it is likely that there will be an increase in diagnostic capacity."

"The topic is certainly not new and has been widely analyzed and what we can say for sure is that no correlation has ever been shown. Specifically, there is a study conducted in Sweden with over 2 million children whose mothers had taken the drug and no connection was found," explained to " Corriere della Sera " the director of the "Mario Negri" Institute for Pharmacological Research and member of the Italian National Health Council.

"It is unlikely that new studies will have the strength to overcome this now consolidated evidence. The sources cited by the 'Washington Post' now speak of a review conducted by researchers at Mount Sinai, but their work has several problems, which the authors themselves highlight. First, the methodology used, which is generally carried out for environmental studies, not in the medical field. This analysis, at best, can provide a basis for discussion, but it is far from being solid scientific evidence," he stressed.

How do you then explain the alarm coming from the US?

Over the years, there have been reports of links between women who had taken paracetamol and children born with autism, but since it is one of the most commonly used medications by pregnant women, it is a coincidental observation that cannot be interpreted as a cause-and-effect relationship.

Can pregnant women continue to use paracetamol by adhering to the prescribed instructions?

The main scientific societies dealing with maternal and child health issues suggest that, if the mother has a fever (an “ally” of the body and not an “enemy”, because it is the body's defensive response to an infectious agent or an inflammatory condition), paracetamol is the most valuable alternative, represented by ibuprofen and aspirin. Of course, like any medicine, it should be taken with great care, but doctors and women themselves know this well. Therefore, to date, paracetamol remains the safest medicine to take during pregnancy.

Precautions for use?

It should not be used for more than five days, a sound rule that always applies to all patients. Furthermore, it is important to remember that, unless specifically indicated by a doctor, no medication should be taken during the first trimester of pregnancy, because this is the period when the organs are forming. However, if there is a real need, paracetamol is the least toxic drug for the embryo at this delicate stage.

Leucovorin, the active ingredient in folic acid, is also being promoted in the US as a possible experimental treatment for autistic children. What do you think?

It is not a cure for autism spectrum disorder. It is a promising approach, but still in the experimental stage, and is effective only for a subset of children, those with antibodies against the folic acid receptor. A 2018 study of over 48 patients showed some benefits: the use of leucovorin improved verbal language and communication skills.

Autism is one of the main concerns of President Donald Trump, who is expressing concern about the growing rate of the condition in the US. What is the situation?

To speak of an increase in the number of cases is absolutely incorrect. However, it is likely that there is an increase in diagnostic capacity: there are new criteria that include many aspects observed that were previously not noticed by doctors and families. Furthermore, the awareness of parents, teachers and social services has increased significantly: this allows early diagnosis and identification of even the mildest forms that were previously not even considered or dismissed as character traits, for example excessive shyness.
